Position Summary
As a Home Care Provider II (HCP II), you carry out the mission of Homebridge to provide high-quality home care to the elderly and people with disabilities, enabling them to live safely and independently in their homes and community.
You will provide services to clients who require more intensive care and oversight. You will implement the client care plans, providing feedback and input to Homebridge staff on client progress, issues, barriers, etc. You will report directly to your Home Care Provider Supervisor.
Essential Job Functions
- Communicates issues or barriers to implementing care plans to assigned Homebridge staff and offer proposed solutions whenever possible
- Uses active listening to gain trust
- Professionally engages in communication with on-site case management and Homebridge staff
- Maintains confidentiality of information about the client and their family, etc
- Keeps appropriate staff informed of any changes in the client’s condition
- Works as a productive, supportive, and communicative team member
- Participates actively in client care conferences/meetings as assigned
- Appropriately refers medical professionals to Homebridge staff
- Recommends ways to improve a client’s safety to Homebridge Care Team
Skills And Abilities
- Basic arithmetic
- Knowledge of basic domestic chores, including cooking and cleaning
- Able to perform personal care services
- Sincere desire to help people in need and provide them compassionate care
- Dependable and able to communicate and follow directions
- Dependable and able to work and travel in all areas of San Francisco
- Ability to demonstrate critical thinking skills and responsible decision making
- Ability to adapt to different personality types and rapidly changing client behavior
- Demonstrate an ability to overcome obstacles to care
- Tailor professional behavior, presentation, and care delivery to suit individual client’s needs
- Understand and implement subtle and specific directions in care plan geared towards the client’s mental health in addition to their physical care
- Understand Motivational Care Management care plan model and work within that model
- Possess strong interpersonal communication skills with the ability to communicate effectively/professionally with medical personnel
- Willing to work with lower-income population, chronically ill, active substance addiction, and low self-compliance with health/medical care plan
- Self-organized can prioritize competing assignments and multi-task
Education And Experience
- Complete the Homebridge Home Care Provider training required
- Experience working with target populations, including people of diverse cultural, education, socio-economic, and linguistic backgrounds, and knowledge of related issues preferred
