Client is a male who lives with his wife. Care will be provided 5 days a week for 4 hours per day. The client is semi-independent with personal care.
Schedule
Monday to Friday, 4 hours per day.
Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support
- Monitor patient health by observing vital signs, changes in condition, and overall well-being
- Prepare nutritious meals tailored to individual dietary needs and assist with meal consumption when necessary
- Support medication administration following care plans and HIPAA privacy regulations
- Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe living space
- Document care activities accurately and communicate updates to healthcare professionals or family members
Requirements
- Prior experience in home health, assisted living, nursing homes, or group homes is highly preferred
- Strong communication skills to effectively collaborate with clients, families, and healthcare teams
- Compassionate attitude with a commitment to providing respectful and person-centered care
- Valid driver’s license and reliable transportation are often required for community-based visits
