Home Care Physical Therapist - 1099

About the role

Seeking a Per Diem Physical Therapist to provide in-home physical therapy care within a designated geographic area. This role offers flexible scheduling and the opportunity to work independently while delivering personalized patient care.

Responsibilities

• Evaluate patients' needs and develop individualized treatment plans
• Implement therapy programs to restore function and promote independence
• Educate family members, caregivers, and team members on therapy techniques
• Document visits accurately and timely
• Communicate effectively with patients, families, and interdisciplinary team members
• Ensure care delivery aligns with policies and professional standards

Qualifications & Skills

• Graduated from a Physical Therapist Program
• Licensed or eligible for licensure in New York
• Mandatory certifications: First Aid, AED, & CPR
• Ability to travel within assigned areas
• Home health experience is preferred but not required; training provided for new graduates

Compensation & Benefits
