Home Care Outreach and Intake Coordinator

Role: Personal Care Outreach Coordinator Responsibilities Conduct community outreach to connect seniors and vulnerable adults with in-home care services.Respond to referrals and educate families and partners about available personal care and support options.Coordinate client intake and enrollment processes.Gather client information, verify eligibility, and ensure accurate documentation in compliance with regulatory and company standards.Collaborate with branch staff and caregivers to schedule services and track outcomes.Support the delivery of compassionate, mission-driven care within the community.
