Our office is located in Central Square, NY—twenty minutes north of Syracuse via I-81. We are licensed in Oswego, Onondaga, Oneida, and Madison counties, with the majority of our clients residing in Oswego and Onondaga Counties.
About the role
Home Care Nurses ensure quality and safe delivery of home health care services that reflect the agency’s philosophy and standards. You will provide one-on-one care to assigned clients in their homes.
Responsibilities
- Ensure quality and safe delivery of home health care services
- Follow the Plan of Care (POC) created by the Director of Nursing
- Implement current nursing practice
- Fulfill additional performance responsibilities as assigned and necessary
- Promote effective written and verbal communication with office staff
- Provide information regarding equipment and supply needs
- Use equipment and supplies effectively and efficiently
- Participate in all aspects of Dignity Plus policies and procedures
- Promote and maintain an agency environment compliant with federal, state, and local regulatory agencies
- Participate in regular in-services and continued education
- Understand that drug and polygraph testing may be conducted at any time
Requirements
- Degree in Nursing (REQUIRED)
- Current and unrestricted Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (REQUIRED)
- Valid Driver’s License (REQUIRED)
- Experience is not required but recommended
- Visual and hearing ability sufficient to comprehend written and verbal communication
- Ability to perform tasks involving physical activity, which may include heavy lifting, extensive bending, and standing

Pay
Hourly rate: $20–$35 per hour, depending on license (LPN or RN) and level of care required for the assigned client.

Schedule
