We are looking for a Registered Nurse interested in providing home care and case management. We are looking for an RN who is interested in the following: Medication set up and oversight, Case management, Tube feeding, Companionship.

Responsibilities

• Provide thoughtful care to clients based on personalized Plan of Care
• Provide in home assessments
• Monitor and track changes in the clients conditions and needs, and keeps the Director of Nursing and any other appropriate personnel informed
• Proactively updates client's family on plan of care, health changes, and concerns

Requirements

• Must have at least 1 year experience
• Current unrestricted RN license in Utah
• Certified CPR Certification
• Negative TB skin test or chest x-ray
• Must be able to pass a drug test and a background check
• Licensed driver with valid automobile insurance
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push and pull
• Must be able to lift 50 pounds
