About The Role
The Home Care Coordinator helps ensure clients receive high-quality, consistent care and exceptional service. Serving as a key liaison between clients, families, caregivers, and the Home Care team, this position coordinates services, supports client onboarding, conducts supervisory visits, monitors satisfaction, and helps maintain individualized care plans.
Schedule: Full-Time | Monday–Friday, 8:30 a.m.–4:30 p.m.
Pay: $22/hour
Responsibilities
- Coordinate the onboarding of new clients, including welcome calls, service orientations, and communication with clients and families.
- Assist with client assessments and the development, implementation, and review of individualized care plans.
- Schedule and conduct supervisory visits to monitor service quality and client needs.
- Monitor client satisfaction and address concerns to support a positive experience.
- Provide compassionate support and guidance while treating every client with dignity and respect.
- Communicate effectively with clients, families, caregivers, and team members to promote continuity of care.
- Support the Home Care Manager with daily operations and client needs.
- Participate in the department’s on-call rotation as assigned.
Requirements
- Associate degree or equivalent experience in healthcare, senior services, social services, or a related field.
- At least one year of experience in home care, healthcare, senior living, case management, client services, or a related field.
- Experience working with older adults; experience with individuals living with dementia is preferred.
- Valid Illinois driver’s license with a good driving record.
- Strong communication, problem-solving, and decision-making skills.
- Ability to work independently while collaborating effectively with a team.
- Strong organizational skills and the ability to manage multiple priorities.
- Computer proficiency.
- CPR and Food Handler certifications required within 30 days of hire.
