Home-Based Primary Care Physician

Responsibilities

• Evaluate and treat chronic conditions and urgent episodic illnesses and injuries
• Order and interpret selected laboratory and diagnostic tests
• Make referrals to specialists as needed
• Prescribe medications
• Perform procedures within the physician’s scope of practice, established protocols, and available equipment

Qualifications

• M.D. or D.O. required
• Current licensure, or eligibility for licensure, to practice medicine in Kentucky, including DEA registration and controlled substance authority, required
• Fellowship training in Geriatrics is preferred but not required
• Board certified or board eligible, with board certification expected within five years of employment, required
• Must be a licensed driver with an automobile in good working order, that is insured in accordance with state minimum requirements
