HIV Non Medical Case Manager

About the role

The Non-Medical Case Management (NMCM) program provides advice and assistance to empower clients to access a variety of necessary services that support medically related outcome measurements. Clients will have varying levels of needs throughout their enrollment, which may include medical, social, community, legal, and financial services. Enrollment in NMCM is not always permanent; clients may transition between types of case management depending on their current situation. Frequent and ongoing assessments ensure clients receive the appropriate level of care.

Responsibilities

• Complete comprehensive assessment of service needs conducted by a non-medical case manager meeting the same qualifications as a medical case manager.
• Coordinate services required to support health outcomes.
• Periodically re-evaluate client needs (housing, transportation, food, legal, emergency financial assistance, and other needed services).
• Encounter clients in their environment, which may include residences, public facilities, streets, or the case manager’s office.
• Inform the client of all Ryan White Part A funded services available and all Ryan White Part A funded case management agencies in the New Orleans Eligible Metropolitan Area (NO EMA).
• Explain what clients can expect if they enroll in case management services.
• Establish and/or verify client eligibility for services.
• Collect required state/federal client data for reporting purposes and complete a full Client Eligibility Review and Certification Form (CERV).
• Complete all Ryan White forms as required by HIPAA, the Grantee, and the agency.
• Develop and complete a Ryan White Plan of Care in collaboration with the client according to their needs and follow up on outcomes.
• Complete referrals for Ryan White services after assessing client needs.
• Upload eligibility documents into the electronic records system within 3 days of eligibility.
• Answer and return phone calls, emails, and faxes in a timely manner.
• Complete all Ryan White billing forms for every client interviewed, including time clock corrections and mileage, on a weekly basis and submit to the Finance Department.
• Work in collaboration with the medical case manager and medical case manager supervisor.
• Ensure recording of health indicators (CD4, viral load, acuity).
• Monitor service utilization, including medical appointment adherence.
• Provide treatment adherence counseling to ensure readiness for and adherence to complex HIV treatments.
• Determine whether a client demonstrates a need or continued need for non-medical case management services.
• Gather ongoing client information to determine eligibility.
• Reassess client status at least every six months or as required to respond to changes, utilizing assessment tools mandated by OHP.
• Adhere to standards of service as outlined in Non-Medical Case Management by the OHP.

Requirements

• High school diploma or GED required.
• Ability to communicate effectively with clients, families, physicians, and others, and fulfill documentation requirements.

Qualifications

• Experience in community health or an official public health agency (preferred).
• Knowledge of HIV disease/AIDS (preferred).
• Knowledge of case management (preferred).
• Bilingual in Spanish (preferred).
