HIV Case Manager

About the role

Provides case management services to patients under the care of assigned providers. Ensures case coordination, effective use of resources and quality of care in accordance with HRSA and Albany Medical Center case management standards.

Responsibilities

• Conducts age-appropriate interviewing to identify patient needs such as adjustment to illness, financial/employment issues, housing, substance abuse, and physical/mental impairments.
• Obtains and evaluates medical, psychological, and social factors contributing to the patient’s situation, considering strengths, limitations, and family context.
• Assesses and responds to persons in crisis, intervening to ensure safety and incorporating legal responsibilities in cases of domestic violence and abuse.
• Provides education on HIV disease, transmission, prevention, and community resources.
• Reviews prevention practices, partner awareness, and notification options; assists patients in developing self-management and health literacy skills.
• Uses motivational interviewing to influence behavior change related to HIV transmission, medication adherence, and other issues.
• Provides cultural and age-appropriate supportive counseling to patients and/or social supports as indicated.
• Counsels patients on adjustment to life-threatening illness, acknowledging diversity and adjusting interventions accordingly.
• Helps patients and families understand, accept, and follow medical and psychosocial treatment plans.
• Assures annual psychosocial assessment via the “Emotional Well-being Questionnaire” at annual comprehensive exams.
• Develops, monitors, and updates service plans annually and as needed, including specific goals and action steps.
• Works with physicians and healthcare teams to address social and emotional factors underlying patient health problems.
• Initiates and updates healthcare proxies/living wills with patients.
• Screens patients for eligibility for entitlement programs, community resources, and internal programs; refers to appropriate services.
• Coordinates case conferences with medical and community agencies; assists with patient retention in care.
• Tracks patient status of Paps, annual comprehensive exams, dental care, and other medical indicators; works with patients and care teams to meet standards.
• Exchanges patient updates and plans with community case managers and provides medical outcome data as requested.
• Communicates patient information to the healthcare team using the EHR tasking feature.
• Refers pregnant patients to family or Part D programs; attends OB case conferences monthly and monitors prenatal and HIV appointments.
• Meets with or contacts newly diagnosed patients at least quarterly during the first year, focusing on retention, adherence, HIV education, and addressing barriers to care.

Requirements

• Bachelor’s degree in psychology, sociology, social work, or related field required.
• Master’s degree in counseling, psychology, or related field preferred.
• Minimum of 1 year of documented experience in case management services for medically compromised patient populations.
• Familiarity with community resources and ability to work in a fast-paced, interdisciplinary team setting.

Skills

• Ability to conduct age-appropriate interviews and assessments.
• Proficiency in motivational interviewing and supportive counseling.
• Understanding of legal issues related to domestic violence, child abuse, and suicidal/homicidal ideation.
• Strong communication skills, including fluency in written and oral English; fluency in Spanish is a plus.
• Knowledge of medical terminology and basic medical information.
• Ability to evaluate patient circumstances and communicate in an objective, nonjudgmental manner.
• Professionalism in addressing interpersonal conflicts and responding to inquiries or complaints.

Pay

$70,068.00 – $108,605.00
