Job Qualifications

• Valid HHA or PCA Certificate
• Valid Photo ID
• Full Physical exam form, PPD, Rubella & Rubeola titers (within 6 months)
• Drug Screen 8-10 panel (within 6 months)
• Two References letters (within 6 months)
