Responsibilities
- Scanning and verifying all hospital departments' paper medical records into electronic medical records.
- Scanning and indexing all inpatient, outpatient, extended care, and emergency room patient reports/documentation into appropriate patient's electronic medical record and storing the paper records.
- Answering and responding to incoming calls in a courteous and professional manner, following Standards of Behavior.
- Assisting other departments in locating/printing electronic records for various reviews and/or studies.
- Completing release of information requests for continuation of care, patient requests, other department requests (Business Office, QI/UM), and after authorization for records (attorneys, insurance companies, etc.).
- Analyzing and completing electronic medical records for outpatient services.
- Verifying all documents are signed either manually or electronically; ensuring the testing/procedure ordered has results/documentation and orders; checking that orders are current, dated, and signed by practitioners.
- Printing unsigned e-sign report and co-signature/verbal order report to verify any electronic signatures needed on medical records and following up with respective practitioners to get their documents e-signed.
- Participating in the completion of department and hospital goals.
- Attending mandatory in-services, department meetings, and other relevant in-services.
- Actively participating in team-oriented decision-making processes and being a team player, willing to be flexible and open to other’s ideas.
- Focusing on daily work towards high customer satisfaction, aiming to meet or exceed customer expectations 100% of the time.

Qualifications
- Demonstrates Clarke County Hospital Standards of Behavior and adheres to hospital policies and procedures and safety guidelines.
- Maintains regular, reliable attendance and demonstrates ability to meet business needs of the department.
- Maintains current licenses and/or certifications required for the position.
- Practices and reflects knowledge of HIPAA, DIA, DNV, OSHA, and other federal/state regulatory agencies guiding healthcare.
- Completes all annual education and competency requirements within the calendar year.
- Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste, and abuse.
- Bridges any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.
- Takes appropriate action on concerns reported by department staff related to compliance.
