Job Description
The Health Information Clerk 11 performs various tasks to support the processing, maintenance, paper and electronic storage, retrieval, scanning and quality review of electronic medical records in accordance with hospital, state, federal, legal, and ISO standards. Responsible for responding to and releasing of protected health information in accordance with regulatory requirements, hospital policies and procedures and federal and state statutes to authorized parties. Maintains integrity of information through the electronic medical record while retrieving confidential patient information. Reviews and evaluates incomplete records and assigns deficiencies (both dictated and electronic signatures). Communicates to physicians and their offices regarding deficient and delinquent records. Prepares records for scanning, verifies patient identification on each page and analyzes for completeness. Scans documents into the electronic document management system and examines pages to ensure image quality and complete quality review indexing (assign form ID). Verifies data integrity. Assures that paper record is disposed of properly. Prepares birth certificates, acquires appropriate signatures and files information. Prepares, properly executes and files paternity affidavits as necessary. Participates as a member of the Health Information Team along with participating in staff development.
Education And Credentials
- High school graduate or equivalent with related courses in typing, filing, office methods and business required.
- Experience 6 months of experience working within a hospital or medical office position preferred.
- Working knowledge of basic computer applications and Microsoft Office suite.
- Knowledge of Medical Terminology, independent judgment in making decisions which may require intense investigation and interpretation in making decisions.
