High Needs Case Manager

About the role

The Care Manager’s main purpose is to ensure their client caseload is provided with the necessary services in a coordinated and effective manner. Responsible for the caring and compassionate delivery of case management, advocacy, and other supportive services to ensure appropriate coordination of services for clients.

Responsibilities

• Create plans and summaries based on client needs.
• Maintain communication, coordination, and connections with clinical team participants.
• Work evenings as needed.
• Prioritize workload in a fast-paced, hands-on environment while demonstrating sound judgment and decision-making.
• Maintain a calm, non-defensive, supportive attitude during difficult interactions.
• Analyze variables and situations, resolve problems, handle conflict, and make effective decisions under pressure.
• Think quickly and effectively in the public spotlight.

Requirements

• High School diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
• One year of case management experience strongly preferred.
• Certification in first aid, CPR, and Crisis De-escalation (training will be offered if not possessed upon hire).
• At least 21 years of age or older.
• Valid driver’s license with acceptable driving record and reliable transportation.
• Prior experience in the behavioral health setting is strongly preferred.
• Obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check.
• Tuberculosis screening test and provide evidence of freedom from infectious tuberculosis prior to providing services; additional TB testing or screening questionnaires will be performed annually.
• Physical, mental, and cognitive ability to perform specific clinical and/or tasks required by the position without posing a direct threat to the safety and security of patients.

Physical demands

• Ability to bend, lift, and carry up to 50 pounds.

• Able to operate Agency vehicle.

Qualifications (Preferred)

• Bilingual—English/Spanish.
• Experience working in Integrated Healthcare, Behavioral Health, or Primary Care environment.
• Advanced knowledge of cultural diversity and sensitivity.
