Responsibilities
- Provides case management services to children enrolled in the behavioral-health system.
- Provides weekly contact with assigned clients, which includes home/community visits, attending medication checks, etc.
- Assists youth in navigating discharge from hospitals, in finding intensive services as clinically indicated, and working to decrease behaviors to improve stability in the home & community.
- Collaborates with other involved agencies, such as Department of Child Safety (DCS), Probation/Parole Departments, Division of Developmental Disabilities (DDD), schools, and other providers.
- Captures and facilitates Child & Family Team (CFT) Meetings.
- Completes referrals, prior authorizations, and other paperwork required by the health plans to enroll clients in services.
- Collaborates with clinical team to ensure that Integrated Service Plans, CFT Meeting notes, and all required paperwork is completed accurately and in a timely manner.
Qualifications
- Mets one of the following education/experience requirements: High School Diploma/GED required and 4 years of Behavioral Health experience. A.A. Degree and two years of Behavioral Health experience Bachelor’s Degree in Social Work, Psychology, or human services field.
- Additional qualifications: Level 1 Fingerprint Clearance Card without Restriction(s) Meet Agency requirements for MVR (Motor Vehicle Record) report Reliable transportation AZ driver’s license AZ auto insurance
