About the role

Discovery at Home is part of the Discovery Senior Living family of companies, a recognized industry leader for performance, innovation and lifestyle customization. The company was established in 2012 with the creation of a Medicare-certified agency in Southwest Florida and has since expanded to Tampa, Florida; Plano, Texas; Houston, Texas; and West Palm Beach, Florida. While primarily serving residents in Discovery Senior Living communities, Discovery at Home provides care services to those living independently.

Responsibilities

• Assist clients with activities of daily living, including bathing, grooming, dressing, and eating
• Provide companionship and emotional support to clients, promoting a positive and friendly environment
• Administer medications, as directed by healthcare professionals
• Aid in mobility and transfers, ensuring the safety and well-being of clients
• Prepare and serve meals according to clients' dietary restrictions and preferences
• Perform light housekeeping duties, such as laundry, cleaning, and organizing
• Accompany clients to medical appointments and other outings, as needed
• Maintain accurate and timely documentation of all care provided

Requirements

• 40 Hr Certificate, Active HHA 75 hour certificate, CNA license (Required)
• Previous experience in a similar role preferred, but not required
• Must have knowledge of Medicare and Medicaid guidelines
• Must have a valid driver’s license and reliable transportation
• Must have a current CPR certification
• Excellent verbal and written communication skills
• Basic computer skill proficiency. Ability to use Microsoft Windows
