Caregiver Responsibilities
- Daily personal care such as bathing, grocery shopping, oral care, cooking meals, etc.
- Aid and provide help with the daily living activities per the Plan of Care
- Track your patient’s condition and/or report to the agency if there is something unusual
- Assist patient with daily meals
- Basic housekeeping functions
- Accompany patient to doctor’s appointments
- Aid with essential errands
- Communicate with Summit regarding any issue that your patient might have
- Follow agency’s policies
Caregiver Requirements
- New York Home Health Aide Certificate or Personal Care Assistant Certificate
- Government issued ID
- Work permit, if needed
- 2 Reference letters
- Recent physical
- Quantiferon Rubella and Rubeola lab work
- Chain of Custody Drug Screen, no older than 6 months
