Responsibilities
- Please note: This is not a hands-on clinical role. This position centers on care coordination and patient education.
- Clinic Days:
- Collaborating with the provider during patient appointments
- Assessing patient responses to treatments and adjusting the plan of care in alignment with goals of care
- Coordinating follow-up appointments, lab monitoring, infusions and other medications, and ongoing care needs
- Managing remote patient care tasks (phone calls, MyChart messages, etc.) as needed throughout the day
- Non-Clinic Days:
- Conducting phone triage to assess patient concerns and determine appropriate next steps
- Responding to patient messages and providing education, guidance, and follow-up
- Evaluating patient response remotely and modifying the plan of care to support goals of care
- Coordinating lab rechecks and monitoring results
Qualifications
- Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system
- Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah
- Maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah
- Bachelor's Degree in Health Care Administration, Health Science or related field or equivalency
- One year of related nursing experience
- Preferred Leadership experience
