Hematology Oncology Nurse Care Coordinator

Responsibilities

  • Please note: This is not a hands-on clinical role. This position centers on care coordination and patient education.
  • Clinic Days:
    • Collaborating with the provider during patient appointments
    • Assessing patient responses to treatments and adjusting the plan of care in alignment with goals of care
    • Coordinating follow-up appointments, lab monitoring, infusions and other medications, and ongoing care needs
    • Managing remote patient care tasks (phone calls, MyChart messages, etc.) as needed throughout the day
  • Non-Clinic Days:
    • Conducting phone triage to assess patient concerns and determine appropriate next steps
    • Responding to patient messages and providing education, guidance, and follow-up
    • Evaluating patient response remotely and modifying the plan of care to support goals of care
    • Coordinating lab rechecks and monitoring results

Qualifications

  • Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system
  • Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah
  • Maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah
  • Bachelor's Degree in Health Care Administration, Health Science or related field or equivalency
  • One year of related nursing experience
  • Preferred Leadership experience
