Helpende Plus Volledig Pakket Thuis Harlingen/Waadhoeke
About the role
As a Helpende Plus / Cliëntondersteuner, you will perform supportive tasks at the client's home. This includes guiding, stimulating, assisting with, and/or taking over personal care tasks such as washing, dressing, putting on elastic support stockings, administering medication, and supervising intake. You will also undertake welfare activities with the client that match their needs and interests, such as reading the newspaper, playing games, walking, outings, and grocery shopping.
The care is provided according to the care plan and under the guidance of the client coach. You will perform household tasks, including cleaning, laundry, and occasionally cooking. You encourage the client's self-reliance and take over where necessary. You observe changes in the client, report these, and coordinate with the client coach. During your work, you can always rely on the client coach or colleagues from home care.
You will work in the Volledig Pakket Thuis (VPT) team of Kwadrant in Harlingen and De Waadhoeke. The VPT team supports individuals with a WLZ indication in their home environment. Many clients have some form of dementia or require structured daily support, assistance with self-management, and autonomy. As a Helpende Plus / Cliëntondersteuner, you are responsible for care, guidance, welfare activities, and household tasks. Since clients have a WLZ indication, you can spend more time and attention on them compared to regular home care.
As a Cliëntondersteuner in Harlingen, you are part of a team consisting of a client coach and several Helpende Plus / Cliëntondersteuners. A district nurse is also involved in the team. Clients live throughout the municipality of Harlingen, and you will visit them using your own car. You will be scheduled during the day, evenings, and weekends.
Responsibilities
- Guide, stimulate, assist with, and/or take over personal care tasks (washing, dressing, medication administration, etc.)
- Undertake welfare activities with clients based on their needs and interests (reading, games, outings, grocery shopping, etc.)
- Perform household tasks (cleaning, laundry, cooking)
- Encourage client self-reliance and take over tasks where necessary
- Observe and report changes in the client’s condition to the client coach
- Coordinate care and support with the client’s network, client coach, and district nurse
- Work independently while collaborating with colleagues in important situations
Requirements
- Diploma Helpende Plus (or willingness to obtain it through Kwadrant’s additional course)
- Own car for client visits
- Digital proficiency (working with tablets or new digital systems)
- Ability to recognize and respond to client needs and interests with initiative
- Strong social skills, empathy, and ability to provide genuine attention while keeping the client central
- Ability to align care and support with the client’s network, client coach, and district nurse
Benefits
- Salary between €2,663 and €3,410 (for 36 hours/week, based on experience and qualifications, FWG 30)
- Permanent contract for 20 hours per week
- 237 vacation hours on a full-time basis (36 hours/week)

Schedule
