HEDIS Stars/Quality Coordinator

About the role

The HEDIS Stars/Quality Coordinator plays a key role in supporting patients through proactive outreach and personalized care coordination. They monitor a specific patient panel, close care gaps, and work collaboratively with providers to help patients achieve both short- and long-term health goals.

Responsibilities

• Collect data and patient records to close HEDIS care gaps and maintain STAR Ratings
• Meet with multiple health plan representatives related to their incentive programs
• Working Provider Portals to close care gaps
• Attest to patient's historical diagnosed chronic and suspect conditions
• Auxiliary support to the Manager in population health programs

Qualifications

• 1 year experience in Quality Care Gaps / HEDIS Stars
• Experience in a Corporate / Health care Setting
• Medical billing is a plus
• MA experience highly preferred but not required
• Ability to work independently as well as in a Team
• This role requires Full Time-In Office Presence
• High level of professionalism
• Excellent written and verbal communication skills
• Excellent organizational and problem-solving abilities
• Ability to work collaboratively with diverse teams and departments
• Knowledge of healthcare regulations and compliance standards
• Strong communication and interpersonal skills
• Proficiency in electronic health records (EHR); eCW preferred, Microsoft products, and other relevant software
• Must have HS diploma or GED equivalent
