About the role
The Novant Health Heart Failure Program is seeking a dedicated Heart Failure Registered Nurse to join the Heart and Vascular Institute Charlotte Team. This position offers the chance to make a significant impact in a dynamic, collaborative environment.
Responsibilities
- Collaborate with the Heart Failure team in all aspects of patient care, managing the care needs of the heart failure population from referral through end-of-life.
- Provide disease self-management education and serve as a resource for heart failure patients.
- Participate in performance improvement initiatives to enhance care, processes, and outcomes.
- Triage patient phone calls and collaborate with medical providers to adjust medications within the scope of practice.
Requirements
- Education: A Bachelor of Science in Nursing (BSN) is preferred, but not required.
- Experience: At least 3 years of clinical nursing experience, with at least 3 years specifically in Telemetry, CCU, or Cardiac Stepdown units.
- Licensure/Certification: Registered as an RN in North Carolina or South Carolina, and certification as a Certified Heart Failure Nurse are both required.
Qualifications
- Demonstrated leadership skills, excellent interpersonal communication, and the ability to effectively communicate patient needs to healthcare providers.
- Strong organizational and critical thinking skills, with expertise in managing heart failure cases.
- Ability to work independently and collaboratively with the interdisciplinary team.
