About the role

At UnitedHealthcare, you’ll wear many hats and work in various environments. Your responsibilities may include:

• Making outbound calls to assess members' current health status
• Taking incoming calls from members
• Identifying gaps or barriers in treatment plans
• Providing patient education to assist with self-management
• Interacting with Medical Directors on challenging cases
• Coordinating care for members
• Making referrals to outside sources
• Coordinating services as needed (home health, DME, etc.)
• Educating members on disease processes
• Encouraging members to make healthy lifestyle changes
• Documenting and tracking findings
• Making 'welcome home' calls to ensure discharged members receive necessary services and resources

Responsibilities

As an RN Maternity Case Manager, you'll:

• Assess members' current health status
• Coordinate care for members
• Make referrals to outside sources
• Educate members on disease processes
• Encourage members to make healthy lifestyle changes
• Document and track findings
• Ensure discharged members receive necessary services and resources

Requirements

• Current and unrestricted Registered Nurse license in state of Florida
• 3+ years of experience in a hospital setting, acute care, direct care experience within the OB and/or Maternity field
• 1+ years of experience working with MS Word, Teams, Excel and Outlook
• Reside in the advertised region

Preferred Qualifications

• Bachelor's Degree
• Case management experience
• Background in managing populations with complex medical or behavioral needs
• Experience with electronic charting
• Bilingual - Spanish
• Experience with arranging community resources
• Field-based work experience
• Knowledge of Florida Community Maternal Child Resources

Soft Skills

• Demonstrated solid organizational skills

Pay

The hourly pay for this role will range from $29.00 to $52.00 per hour based on full-time employment.

Schedule

This is a work from home position with a Monday to Friday 8am-5pm schedule.

Qualifications

To apply, you must meet the following qualifications:
