About the role

Provides nursing care to patients in their homes under the supervision of a licensed nurse.

Responsibilities

• Care for patients in accordance with medical orders and nursing protocols.
• Assess patient’s condition and document findings accurately.
• Administer medications as prescribed.
• Provide education to patients and families regarding home healthcare management.
• Coordinate with other healthcare providers as needed.

Requirements

• HCHB, Oasis, and HH experience required.
• Homecare Homebase Software Experience - Required.
• OASIS Experience - Required.
• Current state license as a Registered Nurse - Required.
• CHPN Certification - Preferred.
• Proof of current CPR certification - Required.
• Current driver's license - Required.
• Must have reliable transportation and insurance - Required.
• 1-2 years experience as a Registered Nurse in a clinical care setting - Required.
• 2+ years Home health experience - Required.

Qualifications

• Graduate of an accredited Associate Degree in Nursing or Nursing Diploma program - Required.
• Bachelors Of Science In Nursing - Preferred.
