Responsibilities
- Conduct inspections, surveys, and investigations of healthcare facilities, service providers, and/or applicants to ensure compliance with standards.
- Evaluate activities to determine the degree to which standards are carried out effectively.
- Identify conditions that do not meet standards and advise personnel on developing plans of correction to remedy deficiencies, prevent reoccurrence, and improve overall quality control and compliance.
- Complete written reports of deficiencies and submit all findings and required documentation in a timely manner.
- Ensure all files pertaining to a survey are secured in accordance with HIPAA and federal/state laws.
- Meet deadlines for written correspondence, documentation, and data entry.
- Adhere to rules regarding agency forms, travel, reimbursement, training, and use of state/federal resources.
- Serve as team leader for survey teams when requested or on a rotating basis, including conducting pre-survey meetings, reviewing findings, and coordinating entrance/exit interviews and travel arrangements.
- Represent the team as spokesperson or liaison during surveys as needed.
- Perform additional duties such as attending local/regional meetings, assisting with staff training, and participating in hearings/trials.
- Participate in disaster preparedness tasks as outlined in the DPH plan, including 24-hour on-call duty during hazardous weather or state of emergency.
Requirements
- High school diploma and relevant program experience.
- Valid driver’s license.
- Ability to plan and coordinate with others, comprehend federal/state regulations, and demonstrate excellent writing and oral skills.
- Proficiency in Microsoft Office and applicable computer software programs.
- Exceptional attention to detail and ability to recognize cultural diversity and incorporate cultural competence principles.
- Knowledge of applicable regulations and policies/procedures.
- Ability to accommodate roundtrip travel of up to 4 hours and 400 miles, with overnight travel 3-4 nights per week.
- Flexible work hours, including occasional weekends, holidays, and exceeding 8 hours per day when needed.
- Physical ability to bend, stoop, lift, and carry up to 20 pounds to a distance/height of up to 20 feet.
- Must become SMQT (Surveyor Minimum Qualification Test) certified, which provides a 5% salary increase.
Any equivalent combination of education and/or experience demonstrating required knowledge, skills, and abilities may be considered, subject to approval by the DPH Chief Human Resources Officer or their delegate.

- Bachelor’s degree in Healthcare Administration, Business Administration, Public Health Administration, Physical Science, Natural Science, or a related field.
