About the role

The Health Care Director (HCD) leads the health and wellness activities for the community’s resident population and manages day-to-day healthcare operations. As the community clinical leader, the HCD manages a team of nurses, medication technicians, and nursing assistants in the provision of quality care and services while ensuring compliance with all applicable local, state, and federal regulations and Harmony Senior Services policies and standards.

Responsibilities

• Evaluates residents prior to move-in to ensure their care and service needs can be met in the community. Conducts reevaluations as indicated by the residents’ condition and/or state regulations.
• Develops a service plan unique to each resident based on the evaluation and the resident’s stated preferences. Revises the service plan as needed based on the residents’ condition and/or state regulations.
• Participates in a review of residents’ service plans with an interdisciplinary team, including the resident, family members, physicians, community leadership, third-party medical providers, and wellness team members to ensure care and service needs are accurately reflected and promote resident safety and satisfaction.
• Collaborates with each resident’s physician, pharmacist, and other healthcare team members to promote coordinated care and services.
• Oversees the community’s medication management program, collaborates with the pharmacy, performs audits, manages physician orders, provides oversight of controlled drugs, and ensures compliance with all state, federal, and local regulations.
• Provides supervision and oversight for medication technicians, Approved Medication Assistive Personnel (AMAP), and other state-approved unlicensed personnel who administer resident medications. Conducts training, skill/competency validation, and medication pass observations in accordance with state regulations.
• Provides supervision and oversight for C.N.A. and other direct care personnel. Offers training as needed to ensure residents’ assessed needs are met.
• Delegates appropriately and within the scope of practice of the nursing license held.
• Maintains a working knowledge of all applicable regulations and ensures documentation meets requirements, keeping the resident care department survey-ready at all times. Participates in developing and complying with plans of correction, as applicable.
• Serves as the community’s infection preventionist, overseeing activities to prevent or limit the spread of communicable infectious illness based on established infection control principles.
• Monitors resident care and quality indicators to identify trends requiring attention and improvement. Discusses findings in weekly At-Risk meetings or monthly QAPI meetings.
• Builds and develops the resident care team by interviewing, hiring, and onboarding new associates, serving as a resource, providing training, evaluating performance, coaching, and fostering a culture of respect and fairness.
• Collaborates with the Executive Director to ensure the resident care department operates within the established budget.
• Collaborates with the Director of Sales and Marketing on external business development opportunities.
• Performs other duties as assigned.

Requirements

• Current valid and unencumbered license to practice as an RN or LPN in accordance with state regulations.
• Completion of any state-required training courses related to practicing nursing in assisted living (e.g., Delegating Nurse course, state-specific medication training).
• Ability to think critically and apply knowledge and principles of the nursing process to promote safe, quality care.
• 1-2 years of experience required; 3 or more years preferred.
• Supervisory/management experience strongly preferred.
• Current CPR certification.
• Ability to work occasional weekends, off-shifts, and take calls after hours as needed.
• Ability to lift, carry, and push up to 50 pounds.

Pay

$76,000 – $78,000 annually
