About the Role

The Health Care Director (HCD) leads health and wellness activities for the community’s resident population and manages day-to-day healthcare operations. As the community clinical leader, the HCD oversees a team of nurses, medication technicians, and nursing assistants to ensure quality care and compliance with all applicable local, state, and federal regulations and Harmony Senior Services policies.

Responsibilities

• Evaluates residents prior to move-in to ensure their care and service needs can be met in the community. Conducts reevaluations as indicated by residents’ conditions and/or state regulations.
• Develops and revises individualized service plans based on resident evaluations, preferences, and changing conditions.
• Participates in interdisciplinary reviews of residents’ service plans with residents, family members, physicians, community leadership, third-party medical providers, and the wellness team to ensure accurate reflection of care needs and resident safety.
• Collaborates with residents’ physicians, pharmacists, and healthcare teams to promote coordinated care.
• Manages the community’s medication management program, including audits, physician order oversight, controlled drug management, and compliance with regulations.
• Supervises and provides oversight for medication technicians, Approved Medication Assistive Personnel (AMAP), and other unlicensed personnel administering medications. Conducts training, competency validations, and medication pass observations in accordance with state regulations.
• Supervises and provides oversight for C.N.A.s and direct care personnel, ensuring training aligns with residents’ assessed needs.
• Delegates tasks appropriately within the scope of the nursing license held.
• Maintains knowledge of applicable regulations and ensures documentation and resident care departments are survey-ready at all times. Participates in developing and complying with plans of correction.
• Serves as the community’s infection preventionist, overseeing activities to prevent or limit the spread of infectious illness.
• Monitors resident care and quality indicators, identifying trends for improvement and discussing them in weekly At-Risk or monthly QAPI meetings.
• Builds and develops the resident care team through hiring, onboarding, training, performance evaluations, coaching, and fostering a culture of respect and fairness.
• Collaborates with the Executive Director to ensure the resident care department operates within the established budget.
• Collaborates with the Director of Sales and Marketing on external business development opportunities.
• Performs other duties as assigned.

Requirements

• Current, valid, and unencumbered license to practice as an RN or LPN in accordance with state regulations.
• Completion of any state-required training courses related to practicing nursing in assisted living (e.g., Delegating Nurse course, state-specific medication training).
• Ability to think critically and apply knowledge of the nursing process to promote safe, quality care.
• 1-2 years of experience required; 3 or more years preferred.
• Supervisory/management experience strongly preferred.
• Current CPR certification.
• Ability to work occasional weekends, off-shifts, and take calls after hours as needed.
• Ability to lift, carry, and push up to 50 pounds.
