About the role
The Health & Wellness Direct Support Professional provides wellness-focused support services to individuals with developmental disabilities. The role engages directly with developmentally disabled adults and their families based on their prescribed individual needs.
Shifts Available
- Monday, Friday, Saturday 8am to 4pm
- Tuesdays 10am to 6pm

- Health and Wellness Coordinator
- Internal: Person Served, Peer DSPs
- External: Families

Summary
The Health and Wellness Direct Support Professional provides wellness-focused support services to individuals with developmental disabilities. The role fosters a teaching relationship that encourages healthy and productive personal growth, community inclusion, creativity, and independence.
Essential Functions
- Absorb and teach individuals about healthy initiatives (including teaching, oversight, and hands-on assistance)
- Encourage individuals to engage in social and community activities as well as appropriate physical/wellness activities
- Communicate effectively with the person receiving services
- Manage crisis and behaviors proactively
- Identify and solve problems for the person served
- Ensure accurate and timely documentation
- Collaborate meaningfully with peers to ensure outcomes are met
- Build healthy habits and maintain a positive attitude
- Aid individuals in choosing activities important to them
- Maintain a safe environment and address safety issues promptly
- Communicate timely with manager regarding pertinent issues and collaborate with peers and other departments
- Model The LADD Way (TLW) ensuring performance is aligned with TLW and department policies
- Represent LADD in a positive manner as appropriate
- Stay updated on relevant training and certifications
Additional Responsibilities
- Participate in annual MyPlan meetings when able
Education, Skills & Experience
- High school diploma or equivalent required
- Current CPR/First Aid Certification
- Current Medication Administration Certification
- Valid driver’s license with fewer than six points, and be insured (if required)
- Ability to navigate various technology software and platforms
Work Environment & Physical Demands
- Work schedule will vary based on individual availability and will primarily include evenings and weekends
- Job may include lifting and other physical activities
- Job will include being in a residential environment and will require travel to other locations
Pay Range
$17.50 per hour
