About the role
The Health System Medication Access Coordinator assists patients in overcoming financial and other access barriers to medications.
Responsibilities
- Evaluate resources for uninsured or under-insured patients
- Ensure adherence to regulatory compliance
- Uphold patient confidentiality and prioritize medication access
- Collaborate with providers, nursing, care management, retail pharmacy, insurance companies, patients, and caregivers
- Connect eligible patients to financial and medication assistance programs and funding
- Support the completion and submission of prior authorizations for prescription medications
- Conduct thorough patient interviews
- Review health and prescription insurance, financial status, and required documentation
- Collect patient information from the electronic health record
- Document activities within the patient chart

Qualifications
- Foundational knowledge of prescription medications
