About the role

The Health Services Coordinator plays a crucial role on the health services team, supporting the department's operations through various administrative and clinical functions. This position requires a strong background in healthcare and a commitment to providing exceptional care.

Responsibilities

  • Conduct and coordinate assessments/evaluations of potential residents and make recommendations for admission.
  • Draft and update Individualized Service Plans (ISPs) as needed.
  • Review service plans with responsible parties and the Executive Director as requested.
  • Preserve dignity and promote independence for each resident.
  • Provide care and services in accordance with service plans and MBK policy and procedure.
  • Train and educate on essential topics such as medication delivery, treatments, safety protocols, and emergency procedures.
  • Perform basic nursing functions like treatments, medication delivery, and managing resident emergencies.
  • Maintain inventory of resident care supplies and minimize waste.
  • Aid in community marketing efforts through positive interactions and serving as a liaison.
  • Maintain a safe and secure environment following safety standards and policies.
  • Make sound, independent decisions and remain calm during stressful or emergency situations.
  • Communicate effectively with personnel, residents, family members, and guests.
  • Organize tasks efficiently and manage time effectively.
  • Assist in transferring residents and managing emergency situations.

Requirements

  • RN, LVN, or LPN License in good standing.
  • 2+ years of prior related work experience in a healthcare environment.
  • Must complete required background clearances, health screenings, and provide negative TB test results.
  • Valid state driver’s license and reliable transportation.
  • Competent and current technical and computer skills, including familiarity with Microsoft Office Suite.
  • Excellent communication skills, including the ability to speak, write, and read English.
  • Ability to make sound, independent decisions and remain calm during stressful or emergency situations.
  • Ability to communicate tactfully and professionally with personnel, residents, family members, and guests.
  • Strong organizational, problem-solving, and time management skills.
  • Ability to move intermittently throughout the workday, in the community, and between neighborhoods.
  • Ability to lift and carry up to 50 pounds and push up to 250 pounds.

Qualifications

  • Must not be an “Excluded Party” as defined by the U.S. Department of Health and Human Services, any state Medicaid Programs, and any additional federal and state government contract programs.
  • Must comply with state licensing regulations, HIPAA, Resident Rights, and other relevant laws and regulations.

  • Knowledge of state and federal regulations.
