Supportive Concepts for Families (SCFF) is seeking a Health Services Coordinator to support our Health Services Department and help ensure individuals with intellectual and developmental disabilities (IDD) receive coordinated, high-quality, person-centered healthcare. This is a field-based nursing role where you'll work independently while collaborating with physicians, specialists, Direct Support Professionals, and interdisciplinary teams to ensure exceptional care for individuals with complex medical needs. Maintain accurate electronic medical records and confidential health documentation. Support nursing teams with physician orders, clinical records, and medical information. Track physical examinations, laboratory results, immunizations, and preventive healthcare requirements. Communicate with physicians, pharmacies, hospitals, families, and residential teams. Support healthcare compliance, audits, reporting, and quality assurance. Qualifications High school diploma or GED required; healthcare-related associate degree preferred. Minimum 2 years of relevant healthcare administrative experience. Experience with electronic health records (EHR/EMR). Strong organizational, scheduling, communication, and follow-up skills. Knowledge of HIPAA, medical terminology, and healthcare documentation. Experience supporting individuals with IDD is a plus!
