Health & Housing Specialist Position in Tempe; BHT/ Case Management Experience; +$3/Hr Differential

The Health and Housing Specialist position is based at the Priest Drive Recovery Health Center in Tempe, AZ (near Loop 202/Priest) and is a full‑time role (40 hours/week) with up to 80% of work performed in the field and 20% in the office, including mileage reimbursement.

Responsibilities

• Provides direct face‑to‑face community‑based services to assist clients to obtain and maintain their housing.
• Conducts screenings, intakes, and assessments with clients to evaluate the nature of their presenting problems in order to determine their needs and develop appropriate treatment plan goals.
• Provides outreach for the purpose of engaging clients into PSH services and orients potential clients to the H20 program.
• Provides transportation to clients as appropriate and as medically necessary, as determined by the CLP team, using personal vehicle or a Terros Health‑provided vehicle when available.
