Position Overview

Care Managers provide outreach and enrollment services to children presumed eligible for New York State’s Children’s Health Home program. They conduct and gather comprehensive assessments to confirm program eligibility and identify areas requiring supportive services. They accurately enter and maintain documentation in accordance with program, regulatory, and organizational requirements. They collaborate with children, families, and service providers to develop, implement, and update a comprehensive Plan of Health Care. They identify, refer to, and coordinate with additional service providers as needed to address identified health, behavioral, and social service needs. They maintain ongoing communication and coordination among families and multidisciplinary service providers to ensure effective execution of the Plan of Health Care. They monitor and document client progress, outcomes, and service engagement on an ongoing basis. They facilitate care transitions as required, including transitions between health homes or from health home services to other levels of care due to changes in eligibility. They advocate for children and families to ensure access to appropriate services and continuity of care.

Key Responsibilities

Qualifications

  • Bachelor’s Degree with 2 years of relevant experience OR a Master’s Degree with 1 year of experience.
  • Valid Driver's License.
