Home Health Care Manager

Responsibilities

• Coordinate skilled nursing, therapy, and personal care visits to deliver safe, high‑quality, patient‑centered care in the home.
• Manage a clinical caseload and develop/update individualized care plans.
• Supervise field staff and collaborate with physicians, families, and community partners.
• Monitor patient outcomes and support compliance with regulatory and accreditation standards.
• Optimize schedules and resources to improve service delivery.
• Foster a culture of respect, communication, and independence for patients.
