Contact Information

Contacts and performs initial interviews with individuals who are eligible for health coaching programs.

Provides necessary coaching to reduce or eliminate behaviors that are considered high-risk.

Develops and conducts seminars and conventions for family members, clients or even employees who are interested in learning more about health and wellness.

Creates and distributes health education materials such as brochures to clients.

Develops care plans that are custom-tailored to individual clients.

Collaborates with clients’ other healthcare providers in order to provide a well-rounded experience.

Identifies the need for behavioral change to improve health status, reduce health risks and improve quality of life.

Examines and records the vital signs like body temperature, blood pressure, pulse rate, weight, height, etc., accurately.

Receives and directs phone calls, schedules appointments, checks-in patients, obtains necessary patient information to file and update patient records, and ensures all forms and consents are completed by patients.

Checks-out patients, assists with referral processing, and arranges laboratory services.

Manages inventory of medical supplies and equips exam rooms with appropriate supplies.

Education and Experience

Possesses at least a bachelor’s degree in a field such as health education, nursing, athletic training or nutrition.

Exhibits excellent knowledge in health, nutrition, and fitness issues.

Knowledgeable about health-related diseases and remedial measures.

Displays excellent verbal and written communication skills.

Displays strong motivational and convincing powers.

Has immense ability to develop a health plan.

Physical Requirements

To perform this job successfully, an individual must be able to perform each essential function satisfactorily.

Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.

The individual must be able to: stand, walk, sit, stoop, reach, lift, see, speak, and hear.

Lifting is limited to 35 lbs. for clinical staff and to 50 lbs. for non-clinical staff.

The individual must use an assisted-lift device or get another individual(s) to assist with the lift that is over these maximum limits.
