About the role

The Health Center Director leads an integrated care model at the 51st Avenue Recovery Health Center in Phoenix, Arizona. This role requires a strong understanding of the SMI population, familiarity with Arizona's RBHA system, and the ability to inspire a multidisciplinary team.

Responsibilities

  • Serves as the administrative supervisor of a Federally Qualified Health Center, overseeing and supervising staff providing services to patients.
  • Evaluates patient care and promotes compassionate, high-quality service.
  • Creates an environment that supports an outstanding, patient-focused healthcare experience.
  • Supervises operational activities to ensure program services comply with policies, procedures, and regulatory requirements.
  • Embraces the recovery philosophy and promotes a safe recovery environment.
  • Directly supervises Clinical Site Managers and other staff.
  • Serves as a subject matter expert in specific areas such as treatment for criminal justice involvement, substance use disorders, suicide risk assessment, and integrated health care models.
  • Holds regular staff meetings and coordinates staffings for difficult cases.
  • Ensures staff meet production requirements and provide effective care coordination.
  • Serves as a liaison among internal departments and collaborates with clinical and medical leadership.
  • Participates in training opportunities and uses clinical and financial data to drive performance improvement.

Requirements

  • LCSW or LPC preferred
  • 3+ years of supervisory experience
  • 5+ years of integrated care or FQHC experience
  • RBHA experience
  • SMI experience required

Qualifications

  • Strong leadership skills
  • Experience with integrated care models
  • Familiarity with FQHC operations
  • Knowledge of SMI and RBHA systems
