Duties and Responsibilities
Engage Member
Conduct outreach to member population to introduce and engage members to participate with Population Health Management team programs.
Receive inbound Care Team phone calls or emails and address or route appropriately.
Assesses the readiness and level of participation of members to engage in care management services.
Refer to RN/program Collaborate with Care Team utilizing appropriate tools to prioritize workload and transfer member to RN if possible, or schedule appointment.
Identify member needs and refer to appropriate internal and external programs.
Redirect providers and members to network facilities when available.

Education Requirements
Requires two years (60 credit hours) of college.
Additional years of qualifying work experience may be considered in lieu of formal education.
Experience Requirements
- Ability to work as part of a multidisciplinary team
- Effective written and verbal communication skills
- 3 years’ experience in a clinical setting, human services, care management or healthcare call center
Work-at-home Requirements
- Must have the ability to provide a non-cellular High Speed Internet Service such as Fiber, DSL, or cable
- Minimum standard speed for optimal performance of 30x5 (30mpbs download x 5mpbs upload)
- Lateny (ping) response time lower than 80 ms
- A dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information
