Health and Wellness Coach

Summary The position is responsible for the delivery of Whole Health Coaching and skill development of the leveland VA Medical Center's Whole Health Coach program - including provision of care through coaching - promotion - education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey - specifically through the Pathway - wellbeing and Whole Health clinical care elements of Whole Health care. Duties Duties Include: Provides individual and group Health and Wellness Coaching to Veterans using Whole Health principles and evidence-based health behavior change strategiesAssists Veterans in identifying what matters most - developing Personal Health Plans - establishing SMART goals - and building skills for sustainable health behavior changeFacilitates Whole Health groups and educational programscollaborates with interdisciplinary teamssupports outreach and engagement activitiesdocuments care in the electronic health recordand assists Veterans with navigation to appropriate VA and community resourcesDuties may be performed across multiple clinical settings and locations based on program and patient care needsWork Schedule: Variable - Monday-Friday - based on agency needs and patient care requirementsoccasional evenings may be required.Virtual: This is not a virtual positionFunctional Statement #:99326S Relocation/Recruitment Incentives: Not AuthorizedPermanent Change of Station (PCS): Not Authorized.
