Responsibilities

• Under the supervision of an RN, assist with Activities of Daily Living (ADL) for participants at the PACE center and in their homes (community, assisted living facilities, rest homes, supportive housing, etc.).
• Contribute to participant care plans through collaboration with the Interdisciplinary Team (IDT).
• Perform non-skilled treatments, including vital signs, transfers, toileting, bathing, and dressing at the PACE center and in the community.
• Assist with recreational activities, including planning individual treatment programs, motivating participation, and evaluating program effectiveness.
• Support registered therapists and certified therapy assistants with participant-specific activities such as positioning, transfers, ambulation, and exercises.
• Maintain a clean and safe environment at the PACE center and in participants’ homes.
• Assist with meal and snack preparation, serving, feeding, and clean-up.
• Use safe techniques in all interactions with participants.
• Document care accurately and timely in the Electronic Medical Record (EMR) and other systems as required.
• Participate in daily cleaning and disinfecting of the PACE center in accordance with CDC guidelines.
• Help distribute and manage health aide tasks to ensure timely completion of participant care and site responsibilities, including community-based and center-based tasks.
• Attend regular team meetings for health aides at your site.
• Follow infection control precautions and CDC guidelines for cleaning and disinfecting.
• Perform all duties in accordance with Fallon Health and Summit ElderCare policies and procedures.

Requirements

• Completion of an approved Home Health Aide or Certified Nursing Assistant Training Program is highly desirable, but equivalent work experience may be accepted in lieu of certification.
• Certification as a Home Health Aide or Nursing Assistant is desired.
• Reliable transportation is required.
• One year of experience working with a frail and/or older adult population, preferably in a PACE program. If under one year, you must complete Summit ElderCare’s training program for health aides before working independently.
• Must complete a standardized set of Summit ElderCare competencies for Health Aides annually.
