About the role

To provide quality services to adults with disability or mental illness, assisting them to become as independent as possible.

Responsibilities

• Transporting individuals to outings in the community, doctors’ appointments, etc.
• Aiding individuals in reaching their set goals.
• Reading and being familiar with the Employee Handbook regarding personnel policies and procedures.
• Assisting individuals in implementing their Individual Support Plans to address their identified goals.
• Documenting services provided in progress notes, including observations, consumer participation, progress, behavior changes, or pertinent information.
• Completing documentation in black ink on the day of service and adhering to company policy regarding transportation and supervision.
• Submitting required documentation by specified deadlines.
• Obtaining and keeping current all necessary training.
• Attending mandatory staff meetings and consumer conferences.
• Reporting any changes in the individual's condition, family situation, or needs to the supervisor.
• Reporting any allegations of consumer abuse, neglect, or exploitation to the supervisor.
• Reporting any destruction or theft of agency property to the supervisor.
• Reporting any possible fraud against the consumer or agency to the supervisor.
• Performing other duties as required by circumstances.

Requirements

• Must be 25 years old
• Must have a High School Diploma or GED
• Must be able to read, write, understand, and follow directions
• Must not have any substantiated findings on the NC Health Care Abuse Registry
• Must meet SBI and DMV requirements of LME/MCO contracts
• Must have a valid driver’s license
• Must have 3 full years of driving experience
• Must have an acceptable level of automobile liability insurance
• Experience with persons with intellectual/developmental disabilities preferred but not required
• Must be willing to complete all required trainings as assigned
• Must have access to a device with internet connection at the time services are rendered
