Responsibilities

• Facilitate the implementation of Individual Service Plans (ISP)
• Participate in administrative duties including staff hiring, training, scheduling, and oversight
• Supervise a team of Caregivers supporting individuals served in the program
• Provide support to individuals receiving services: assistance with daily activities such as meal preparation and planning; medication administration; and personal care
• Duties split between providing direct support, professional or program activities, and supervision
• Facilitate community involvement by accompanying individuals on outings in the community or providing transportation to work or other activities

Requirements

• Valid driver's license with one (1) year history and no restrictions
• Current car registration and auto insurance
• Must be 18 years of age or older

Qualifications

• High school diploma or equivalent
• One year of related work experience

Skills

• Excellent communication skills with ability to establish rapport with team members and those served
• Strong organizational abilities to ensure staffing and schedules are maintained
