Group Home Coordinator (Designated House Coordinator)

Responsibilities

• Provide on-site supervision and leadership to staff at the group home or residential location.
• Ensure compliance with 245D regulations and Fraser policies.
• Support active treatment service development and implementation.
• Lead continuous quality improvement initiatives at the location.
• Supervise staff and support their professional development.
• Serve as a primary point of contact for clients, families, and community partners.
• Maintain accurate client and staff records, including documentation required by regulations.
• Participate in emergency response and preparedness activities as needed.

Requirements

• HS Diploma or Equivalent AND four (4) years of full-time work experience (8,000 hours) providing direct care services to persons with disabilities OR nursing/assisted living home experience.
• Bachelor's Degree in Human Services or related field AND 1 year of full-time work experience providing direct care services to persons with disabilities.
• Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities.

• Candidates must qualify based in one of the following three ways, as assigned by 245D, which includes a combination of work experience and education.

Location, Schedule & Pay

• Location: Bloomington, MN, Plymouth, MN
• Schedule: Full time Monday-Friday, 9 AM - 5 PM
• Starting pay range: $21.50 - $23.50 with an additional 2-dollar differential per hour depending on up qualifications and location
• Hiring Bonus for external applicants: $1,000
