Pay

$15.00 per hour

Schedule

Every Thursday and Friday 3 p.m.–10 p.m.; every other weekend (Saturday & Sunday) 8 a.m.–8 p.m.

Responsibilities

• Deliver support per individual service/program plans, including socialization, health maintenance, medication administration, and skill/behavioral development.
• Assist with daily activities such as personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.
• Provide direct support in accordance with service plans; document progress and maintain confidentiality.
• Make cost‑effective purchases within the individual’s budget and preferences.
• Maintain professional relationships with individuals, families, guardians, and case managers; greet visitors and follow Sevita Customer Service Standards.
• Report alleged abuse, neglect, medical, behavioral, and other incidents per internal and external policies.
• Respect and uphold the rights of persons served, practice universal precautions, and assist individuals in exercising their rights.
• Accompany individuals to medical appointments as needed and relay orders/information to providers.
• Accurately administer or support self‑administration of medication, document delivery, report errors, and secure controlled substances.
• Monitor health, document concerns, communicate with nursing staff, and call 911 in emergencies.
• Utilize medical equipment correctly and report malfunctions.
• Prepare or assist with food preparation according to menus, special diets, and individual choices; monitor mealtime.
• Attend orientation, ongoing training, and monthly staff meetings.
• Work scheduled hours, arrive on time, stay on shift until replacement arrives, and notify supervisors of absences promptly.
• Collaborate respectfully with team members, communicate openly, and uphold workplace safety policies.
• Drive safely when transporting individuals, use seat belts, lifts, and wheelchair ties, and report accidents or safety concerns immediately.
• Perform daily and seasonal housekeeping, maintain a clean and safe environment, and report hazards.
• Conduct minor repairs, change light bulbs, perform yard maintenance, and report larger maintenance needs.
• Check water temperature when assisting with bathing and participate in safety drills.
• Perform other duties as required.

Requirements

• High school diploma or equivalent (preferred).
• At least 18 years of age.
• Six months of experience in human services (preferred).
• Working knowledge of computers.
• Valid driver’s license in good standing (must remain valid for at least one year after permit) and clean motor vehicle record meeting Sevita insurability policy.
• Car registration and vehicle insurance if providing transportation.
• All state‑required training(s) and certification(s) completed within mandated timeframes.
• Drug testing prior to employment and random/drug‑testing for cause.
• Criminal background check and clearance by the Department of Human Services.
• Name check against the Central Registry of Offenders Against Individuals with Developmental Disabilities and the Child Abuse Registry Information (CARI).
• Completion and demonstrated proficiency in all required trainings.

Work Environment

Work is performed at the program location and may include accompanying individuals into the community. Physical demands include heavy work, exerting up to 100 lb of force occasionally, up to 50 lb frequently, and up to 20 lb constantly to move objects.
