About the role
Graduate Practical Nurse position in a skilled nursing and rehab setting.
Responsibilities
- Provide compassionate bedside care under RN supervision.
- Assist with assessments, medication administration within scope, wound and restorative care, ADL support, and documentation in the EMR.
- Collaborate with an interdisciplinary team to implement personalized care plans.
- Monitor resident conditions and report changes promptly.
Qualifications & Skills
- Current practical nurse licensure or certification.
- Strong communication, teamwork, and compassionate patient care skills.
- Ability to follow detailed care plans and document accurately.
- Willingness to learn in a supportive environment focused on dignity, safety, and quality outcomes.
