About the Role

The Global Patient Services Care Manager serves as a single point of contact for special populations, including Military, International, and Worker's Compensation patients, families, external Case Managers, referral sources, and payers. This role ensures access to services and maintains ongoing communication throughout treatment by providing concierge services that address medical requirements, cultural nuances, language barriers, visa requests, orientation, family support, and other unique needs associated with travel from other regions or countries.

Responsibilities

  • Lead treatment planning to address unique needs such as return-to-work goals, embassy requirements, international discharges, length of stay (LOS), home modifications, and life-care planning.
  • Manage communication with external Case Managers, Claims Adjusters, Medical Attachés, and Embassies, keeping referral and payer representatives informed of patient progress and ongoing needs.
  • Secure pre-authorizations, embassy approvals, and letters of compensability prior to admission.
  • Work with Patient Financial Services and Managed Care to negotiate self-pay agreements, payer discounts, and single-case agreements with third parties.
  • Manage and disseminate information on local housing, air ambulances, ground transportation, and other travel resources for out-of-state families.
  • Coordinate interpreter and translation services for International Patients.
  • Coordinate care with external providers and partners (including NMH, CMH, UCMC, and private physician groups) to ensure smooth transitions.
  • Participate in quality improvements for individual, team, and program performance.
  • Perform additional duties as assigned in the best interest of the Shirley Ryan AbilityLab.

Requirements

  • Master’s degree for Social Workers or CRRN for Nurses.
  • Current Illinois license.
  • Minimum of 2 years of progressively responsible related clinical work experience with the diagnostic program population.
  • Experience with International, Worker’s Compensation, or Military Populations preferred.

Skills

  • Professional knowledge of practice and supervision.
  • Interpersonal skills sufficient to provide effective communication with patients, families, and staff.
  • Sophisticated conflict mediation, negotiation, and consensus-building skills.
  • Highly developed referral relations skills and the ability to promote services to external customers.

Working Conditions

Normal office environment with little or no exposure to dust or extreme temperatures.
