About the role
The Clinical Case Manager for Gilead’s Middletown area adult supported apartment program works M, T, W, Th, 3p-11p, Every Other Weekend 3p-11p providing case management, advocacy, educational, and supportive services in a program serving adults with major mental illness and co-occurring substance use disorders.
Responsibilities
- Provide case management, advocacy, educational, and supportive services in a program serving adults with major mental illness and co-occurring substance use disorders.
- Carry out all phases of clinical services including intake, engagement, assessment, treatment planning, case management, referral, and discharge.
- Timely completion of required documentation including assessments and other treatment planning.
- Work with the treatment team to manage clients’ mental health and assess for risks, implementing risk management plans as necessary.
- Provide recovery-oriented clinical case-management services with an emphasis on dignity, respect, and inclusion of family and other natural supports.
- Provide support, skills training and education in areas including daily living skills, nutrition, financial management and symptom management.
- Monitor positive outcome measures and ensure program compliance through accurate and timely data reporting.
- Complete assessments, treatment plans, and other documentation electronically and on paper in a timely manner.
- Act with compassion, integrity, and ethics.
Requirements
- Bachelor’s degree in a human service field; Master’s degree preferred
- One year of clinical experience
- Maintain and provide proof of valid driver’s license and automobile insurance in good standing
- Reliable use of personal vehicle as needed
- Experience with co-occurring psychiatric and substance abuse disorders preferred
Qualifications
- Minimum requirements include a Bachelor’s degree in a human service field; Master’s degree preferred

- Training and education in clinical knowledge

Pay
$20.83 per hour
