Geriatrics Nurse Practitioner

About the role

Signify Health, part of CVS Health, is hiring board‑certified nurse practitioner contractors to conduct In‑home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. The visits include a medication and medical history review and a physical evaluation performed in the health plan member’s home. Contractors receive a pre‑populated, patient‑specific form for each evaluation and do not prescribe medicine, order lab tests, or alter the member’s current treatment regimen.

Responsibilities

• Conduct in‑home health evaluations (medication and medical history review, physical examination).
• Use the provided pre‑populated, patient‑specific form for each visit.
• Refrain from prescribing medication, ordering lab tests, or changing the member’s existing treatment plan.
• Engage with members one‑on‑one to help them re‑engage with their health.

Requirements

• Current board‑certified Nurse Practitioner credential.
• Active NP license in the state(s) where services are provided.
• Ability to work flexible hours and manage other personal or clinical commitments.
• Area exclusions apply.

Skills

• Strong interpersonal skills for connecting with people one‑on‑one.
• Ability to educate members and assess their understanding of treatment plans.
• Comfort performing medication reviews and physical evaluations in a home setting.
