About the role
Ascension Living PACE - Michigan is seeking a Geriatric Nurse Practitioner to join an established outpatient team of 3 Physicians and 2 Advanced Practice Providers. This full-time, employed position offers clinical autonomy, a collaborative interdisciplinary practice, and a manageable caseload of approximately 80 patients with a daily volume of 2–6 visits, allowing meaningful time to build longitudinal patient relationships.
Schedule
- Full Time, Monday–Friday, 8:00 AM–4:30 PM

Requirements
- Master’s degree in Nursing
- Registered Nurse license obtained prior to hire
- Nurse Practitioner certification obtained prior to hire
- BLS Provider certification (American Heart Association or American Red Cross) obtained within 1 month of hire
