Geriatric Nurse Practitioner

About the role

We are seeking a compassionate Nurse Practitioner to provide high-quality, patient-centered primary care to residents in assisted living and long-term care communities. This role is designed for a provider looking to focus on meaningful patient relationships and clinical excellence without the burnout of hospital shift work. Working alongside a supportive interdisciplinary team, you will manage chronic conditions, perform comprehensive assessments, and deliver specialized geriatric care in a supportive, collaborative environment.

Responsibilities

• Conduct comprehensive physical exams, health assessments, and routine medication reviews.
• Formulate and execute personalized care plans in collaboration with the interdisciplinary clinical team.
• Diagnose and manage acute and chronic conditions common in the geriatric population.
• Order, perform, and interpret diagnostic tests and laboratory work.
• Prescribe medications and therapies responsibly within the New York State NP scope of practice.
• Educate residents, family members, and facility staff on health maintenance, disease prevention, and wellness.
• Maintain complete, accurate, and timely electronic health record (EHR) documentation in eClinicalWorks.
• Participate in facility-wide quality improvement initiatives and care coordination efforts.

Requirements

• Master's Degree in Nursing (MSN) from an accredited Nurse Practitioner program.
• Active, unrestricted New York State Nurse Practitioner (NP) License and active DEA registration.
• Actively credentialed and enrolled with both Medicare and Medicaid in New York State.
• Minimum 3,600 clinical practice hours completed in accordance with NYS independent practice regulations.
• Minimum 3 years of clinical primary care experience.

Preferred Qualifications

• Prior experience in Geriatric Primary Care, Assisted Living, or Long-Term Care settings.
• Strong proficiency with electronic health records (eClinicalWorks preferred).
• Strong clinical assessment skills, excellent bedside manner, and effective communication skills.

Schedule
