About the role
We are seeking a compassionate Nurse Practitioner to provide high-quality, patient-centered primary care to residents in assisted living and long-term care communities. This role is designed for a provider looking to focus on meaningful patient relationships and clinical excellence without the burnout of hospital shift work. Working alongside a supportive interdisciplinary team, you will manage chronic conditions, perform comprehensive assessments, and deliver specialized geriatric care in a supportive, collaborative environment.
Responsibilities
- Conduct comprehensive physical exams, health assessments, and routine medication reviews.
- Formulate and execute personalized care plans in collaboration with the interdisciplinary clinical team.
- Diagnose and manage acute and chronic conditions common in the geriatric population.
- Order, perform, and interpret diagnostic tests and laboratory work.
- Prescribe medications and therapies responsibly within the New York State NP scope of practice.
- Educate residents, family members, and facility staff on health maintenance, disease prevention, and wellness.
- Maintain complete, accurate, and timely electronic health record (EHR) documentation in eClinicalWorks.
- Participate in facility-wide quality improvement initiatives and care coordination efforts.
Requirements
- Master's Degree in Nursing (MSN) from an accredited Nurse Practitioner program.
- Active, unrestricted New York State Nurse Practitioner (NP) License and active DEA registration.
- Actively credentialed and enrolled with both Medicare and Medicaid in New York State.
- Minimum 3,600 clinical practice hours completed in accordance with NYS independent practice regulations.
- Minimum 3 years of clinical primary care experience.
Preferred Qualifications
- Prior experience in Geriatric Primary Care, Assisted Living, or Long-Term Care settings.
- Strong proficiency with electronic health records (eClinicalWorks preferred).
- Strong clinical assessment skills, excellent bedside manner, and effective communication skills.
Schedule
