Geriatrician

About the role

Provide comprehensive, patient-centered care for older adults in outpatient and community settings.

Responsibilities

• Conduct multidimensional assessments
• Manage chronic conditions and optimize medications
• Diagnose and treat cognitive and behavioral health issues
• Facilitate advance care planning
• Provide consults
• Perform home visits and transitional care in community settings
• Coordinate with interdisciplinary teams to address medical, functional, and social needs

Requirements

• MD or DO
• Board certification or eligibility in Geriatric Medicine
• Active Massachusetts medical license
• Experience with complex older adults

Preferred Qualifications

• Home-based care experience
• Palliative and dementia care experience
• Telehealth experience
• Quality improvement experience
