Geriatric Physician

Position Overview

A Board‑Certified Physician is sought to join a geriatric medicine team serving skilled nursing facilities, providing high‑quality elder care with full scheduling autonomy. The role involves comprehensive medical care for long‑term and post‑acute patients, including assessments, medication management, care planning, and collaborating with a team of Geriatric Nurse Practitioners.

Key Responsibilities

• Conduct comprehensive assessments of long‑term and post‑acute patients.
• Respond promptly to acute changes in patient condition.
• Supervise and mentor team members, including Geriatric Nurse Practitioners.
• Complete all documentation in the EPIC EMR system.
• Participate in interdisciplinary meetings.
• Communicate effectively with patients, families, and facility staff.

Qualifications

• Completion of an ACGME‑accredited residency.
• Board Certification in Family Medicine or Internal Medicine.
• Active medical licenses in Wisconsin and Illinois.
• DEA certification.
• BLS certification.
• Preferred: Experience in post‑acute or long‑term care and geriatric medicine.
